Enhanced Access

Charnwood GP Network host the Enhanced Access Service for both Beacon and Carillon PCN’s, providing 67.5 hours of additional appointments per week. Enhanced Access Appointments are available 18:30 – 20:00 Monday to Friday, and 09:00 – 17:00 on Saturday. These appointments are a variety of remote consultations and face-to-face appointments and mainly hosted at Rosebery Medical Centre, Loughborough on behalf of the entire PCN. If you would like to book an appointment in the Enhanced Access Clinic, please contact your GP Practice.

Spirometry

Charnwood GP Network provide breathing test clinics and offer Spirometry and FeNo testing for patients registered with a Shareholder Practice, the patient can be referred by their GP Practice and clinics are held on a Saturday at Rosebery Medical Centre, Loughborough and Charnwood Surgery, Mountsorrell.

Pre-Transfer Clinical Discussion and Assessment (PTCDA)

The Pre-Transfer Clinical Discussion and Assessment (PTCDA) pilot, launched on 31 March 2020, was a clinically-led initiative involving collaboration across primary, community, secondary care, ambulance services, and social care. Accelerated by the pandemic, it focused on rapid clinical discussions—often with geriatric specialists—to avoid unnecessary hospital admissions and promote safer, community-based alternatives.

Patients benefited from enhanced community assessment bundles, with follow-up visits by geriatricians when needed. Those admitted to hospital received active case management, reducing their stay, and continued structured care upon return to the community.

The PTCDA pilot can now be accessed by any community-based clinician who is considering admitting a patient with significant frailty/complex comorbidity, whether from a care home or their own home. This includes East Midlands Ambulance Service (EMAS) paramedics and technicians, general practitioners and other practice-based clinicians, and Derbyshire Health United (DHU) out-of-hours professionals.
